Every hazard gets an owner,
and a verified close.

When WSafe AI sees a hazard and a competent person confirms it, HazardLoop routes it, escalates it, and carries it across shifts until it is fixed and verified.

Where HazardLoop sits

The action-and-closeout stage of the WorkSafeAgent loop.

HazardLoop is the step that turns a confirmed detection into an owned, verified fix — and feeds the result back so the next detection is better.

How the loop runs

One confirmed hazard. One owner. One verified close.

01Intake

A ticket opens only after a person confirms.

When WSafe AI flags a hazard, a competent person must confirm or dismiss before HazardLoop opens a ticket. Dismiss reasons are kept as training labels that improve the next detection.

Example

WSafe AI flags a missing guardrail on Level 7. The site supervisor opens the clip, confirms, and HazardLoop opens Ticket #HL-00142.

02Route

Right competent person, right site.

Each ticket is routed by area and trade to a named owner. InductIQ confirms the crew assigned to the fix is inducted and qualified for that work. No forwarding chains, no group chats.

Example

Ticket #HL-00142 routes to the Level 7 Structural lead; InductIQ confirms the two crew members hold current working-at-height inductions.

03Escalate

Acknowledgment timers and paging up a ladder.

Every ticket carries an acknowledgment timer. If the owner does not acknowledge, the ticket pages up a defined ladder of contacts until it is picked up. The full timeline is on the ticket.

Example

Acknowledgment timer reaches 15 minutes with no response; HazardLoop pages the project superintendent, then the HSE manager.

04Shift handoff

Open items carry across shifts.

At shift change, open tickets are handed to the incoming competent person for that area and trade, with context, evidence to date, and the current state preserved.

Example

End of day shift: Ticket #HL-00142 is still 'In progress' and is handed to the night supervisor with the before-photo and route notes intact.

05Verified closeout

No ticket closes without proof.

Open → Acknowledged → In progress → Abated → Verified. Verified requires before/after evidence, a same-view camera re-check, and competent-person sign-off. The crew re-brief for that fix is recorded in InductIQ.

Example

Guardrail reinstalled; HazardLoop pulls the same camera view used in the original detection, the supervisor signs off, and InductIQ logs the re-brief for the crew.

06Learning loop

Every close teaches the next one.

Closed tickets become WorkSafeStory lessons. Every confirm, dismiss and closeout feeds back into WSafe AI so detection keeps improving over time.

Example

Ticket #HL-00142 becomes a 2-slide WorkSafeStory lesson 'Guardrail re-install after lift drop' and tunes WSafe AI for the same view.
